The Anterior Retroperitoneal Approach to the Degenerative Lumbar Spine: Clinico-Therapeutic Remarks
نویسندگان
چکیده
The anterior approach is a well-known surgical technique for the treatment of degenerative diseases of the lumbar spine. Through this approach is possible to perform arthrodesis, arthroplasty, discectomy, corpectomy to treat a lot of degenerative conditions. The ALIF procedure is one of the most widely used technique, and is argued to have biomechanical advantages over posterior approaches for lumbar spinal fusion: restoration of disk height and lumbar lordosis, reduction of listhesis, restoring of coronal and sagittal balance. Through this approach is also possible to perform a lumbar disc arthroplasty, a motion preservation technique which aim is to restore the biomechanical properties of the lumbar spinal motor unit. Moreover, in a more recent period, a lot of alternative anterior approaches are described and used, such as Extreme Lateral trans-psoas approach (the XLIF technique). In the current clinical practice, anterior approaches seem to play an extremely important role for the management of lumbar degenerative diseases. In this editorial we want to describe systematically the classic anterior approach, highlighting advantages and disadvantages of the surgical technique and related complications based on personal experience and literature review. Aim of this article is to provide an updated window on the surgical technique for the anterior retroperitoneal approach, frequently used for the treatment of degenerative pathology of the lumbar spine.

Video-Assisted Anterior Retroperitoneal Approach to the Lumbar Spine. A Minimal Invasive Technique Improved by the Use of Endoscopic Camera to Treat Lumbar Spine Diseases
The anterior retroperitoneal approach is a technique to treat disc degenerative disease (DDD), spinal deformities, traumas, tumors and infections. It can be used to perform Anterior Lumbar Interbody Fusions (ALIF) or Total Disc Replacements (TDR). Though being a fast procedure that is becoming more often used by spinal surgeons, the anterior approach requires an adequate knowledge of the anatom...

STUDY DESIGN A morphometric analysis, using magnetic resonance imaging (MRI) studies of the lumbar spine. OBJECTIVE To identify the anatomic position of the ventral root and the retroperitoneal vessels in relation to the vertebral body in normally aligned and deformed spines. SUMMARY OF BACKGROUND DATA The lateral approach to the lumbar spine is a relatively new method for performing interb...
متن کاملRole of Minimally Invasive Spine Surgery in Adults with Degenerative Lumbar Scoliosis: A Narrative Review
Background and Aim: Degenerative lumbar scoliosis is a spinal deformity resulting from advanced disc degeneration and facet arthropathy. Given the inconclusive available literature and lack of high-quality data supporting the role of minimally invasive surgical management of degenerative lumbar scoliosis, this review intends to highlight and compare the various viable minimally invasive surgica...
متن کاملExtreme Lateral Interbody Fusion (XLIF): a novel surgical technique for anterior lumbar interbody fusion.
BACKGROUND Minimally disruptive approaches to the anterior lumbar spine continue to evolve in a quest to reduce approach-related morbidity. A lateral retroperitoneal, trans-psoas approach to the anterior disc space allows for complete discectomy, distraction, and interbody fusion without the need for an approach surgeon. PURPOSE To demonstrate the feasibility of a minimally disruptive lateral...
متن کاملTwo-year clinical and radiographic success of minimally invasive lateral transpsoas approach for the treatment of degenerative lumbar conditions
BACKGROUND The lateral transpsoas approach to interbody fusion is a less disruptive but direct-visualization approach for anterior/anterolateral fusion of the thoracolumbar spine. Several reports have detailed the technique, the safety of the approach, and the short term clinical benefits. However, no published studies to date have reported the long term clinical and radiographic success of the...
